PWP = Personal Wellness Profile
A personal wellness profile is an educational report designed
to help you evaluate your current lifestyle, identify health
risks, and decide where and how to make improvements.
Executive Summary
The executive summary is a report that summarizes the primary
health findings for those individuals who completed a Personal
Wellness Profile (PWP). Group health needs and risks are presented
along with recommendations for initiating risk reduction and
health enhancement programs. This information is very helpful
in planning a comprehensive health enhancement program.
Group Report
A group report describes how all the participants tested are
doing. It shows the prevalence of risk reductions in both numbers
and percent and specifies how many in the group are in the low-to-high-risk
categories.
Options available at Mercy only:
- Functional Capacity Evaluation
Pre-Job Offer evaluations can be completed based on essential
functions and job descriptions provided by facilities. This evaluation
can assist with determining a participant's ability to perform
job tasks.
