Transitional Care

Transitional care is a 30-bed subacute and skilled care nursing facility providing 24-hour nursing care. The customer is assessed upon admission and continuously thereafter as to their medical needs using a multi-disciplinary approach. Multiple medical interventions are utilized expeditiously to optimize the condition of the customer for discharge to the next appropriate setting.
